What a notice needs

Your name, address, phone and email; the copyrighted work; the exact CLIP OR NOT URL or clip id; a statement of good-faith belief that the use is not authorised; a statement that the notice is accurate and, under penalty of perjury, that you are the owner or authorised to act; and your physical or electronic signature.

Counter-notice
